Critical human factors, i.e., the interface of humans with other humans, the equipment being operated, and the related policies and procedures are typically identified during a formal incident investigation following a significant process safety incident. The challenge is to proactively identify and effectively manage the most critical human factors, in order to avoid serious process safety incidents.
